I think it has to be with the "Auto center new layers" option. With this option on, any new layer you create will be centered to the current camera position.
Personally, I prefer to uncheck this feature, because it's easy to get issues like the one you are showing. I recommend you to go the Preferences, Layers/Object tab and uncheck "Auto center new layers".
That will avoid the problem in future documents.
Now, to fix the already made documents, you must select each layer and check its Z position (with the Transform layer tool). In a normal document, the layers' Z value should be 0.
Sadly, once you set Z to 0, you will have to manually fix the position and scale of your object to fit with what you need.
To have a clearer idea of what is happening, go to "Display Quality" and uncheck "Masking". Then use the orbit tool to see if all layers are in the same Z position.
PD: If the camera were Panned or Tilted, this option can also affect the X and Y rotation of new layers. Maybe you will have to check the values in the "Rotate Layer XY" tool. In a normal document, X and Y should be 0.

I think Auto Center New Layers trips up everybody that's just starting out (I had posted a similar question a couple of years ago) and it sounds like most users wind up disabling it.
I wonder if it shouldn't be disabled by default. I've only needed to re-enable it once in all this time and I immediately disabled it after I was done with my task.

The idea of auto-centering new layers had to do with camera movement. For example, if you pan the camera way to the right and then add a new layer, people were wondering why the layer's origin was way off to the left. If you then rotated or scaled that new layer, the center of transformation would be somewhere offscreen.
I guess it's a matter of two different kinds of potential confusion. If you guys think there would be less confusion with this off by default, maybe that's what I should do for 11.2.

Probably should take a poll but, yeah, to me it's more confusing when it's on.
For the longest time, I thought ASP was being buggy whenever I created an new element and I had to keep re-positioning things relative to the origin manually. It made me nuts until somebody in this forum showed me the option to disable this behavior.

OMG! I think this "auto center new layers" option has been tripping me up for years. I would move an object in to group and it would shrink. So I would resize and move it.
But things would never look or behave right.
