That's an interesting Smart Bones setup. I'm guessing this is what you expected...

This is a technique I like to call Daisy Chaining. I'm using the blue (Both) Smart Bone to operate the green (Left) and orange (Right) Smart Bones.
When you set this up yourself, you're going to find that the blue (Both) Smart Bone works fine on frame 0, but doesn't work on an
animation frame because Moho will ignore the daisy chain when there are multiple bones involved. This is not a bug; Moho does this to optimize the UI's performance.
However, you can force Moho to respect the setup by targeting a dummy bone at the controlling Smart Bone. In this case, I have a bone called Activator (the small red bone) targeting the blue (Both) bone.
In very complex setups, I'm guessing this setup might impact performance, but I've never noticed a problem with it in my setups.

Tip: After setting this up, you should Shy the 'activator' bone. There is no reason to animate this bone, so you may as well 'hide' it. It's good practice to Shy any bones that are not meant to be keyframed on the Mainline, especially when that can break the intended operation of the rig.
